Best advice I could give you is speak with the manager of the bank through which your card is issued. You do not need to reveil with whom the charge or company you create the debt.
Just supply her/him with the amount, the dates of charges and the phone number. You will find these on your bank statement or if you do online banking, they are provided there, as well.
The bank can do nothing to refund your money, for that you will have to go through other channels.
However, request your card be cancelled and a new one be issued.
If you find it too unconfortable to speak with the manager, a quick call or speaking to any teller will complish the same. You could just say you misplaced it & for security reasons you would prefer a new number.
